Problems with layers of varnish, paint and primers.
There are several causes for loosening of varnish, paint and primer from the underlying layers or from the stretcher, for example by mutation of moisture through the canvas and the layers of primer and paint. Another example might be shrinkage of the panel, causing the old layer of paint and primer, having lost its flexibility, to be squeezed together and come loose from the panel.
